Open Art Talent Sign Up

This form is for talent signing up to take the stage at the Open Art Series. If you want to perform and multiple events, please complete a different form for each event (Act). If you are a group, only the rep needs to complete the form, but list each member in the Group Member section. Acts (group or solo) only have to donate $5. Your donation helps the success of the event.

All acts must be there by 7 pm, or their slots may be replaced. All acts have 5-minute sets. If you need to send music, please send it to support@daressiaindustries.com SUBJECT: "Show Date Music" and "Name"
